Community Health Initiative (CHI) offers health insurance enrollment and renewal assistance for Covered California, Private Individual and Family Plans, Medi-Cal, Medicare, along with dental and vision plans! We also conduct outreach and utilization services.
Once our Health Access Specialists have assisted you in enrolling in the appropriate plan, they will follow up with you to make sure your application is approved, that you receive your insurance plan cards, and that you are able to access services. We will contact you twice a year to survey your satisfaction and usage of the insurance plans. When the time comes for you to renew your eligibility, they will assist you in completing the appropriate forms and submitting all required documentation.
