COVID-19 Resources

Health Plans have waived the cost for members to receive COVID-19 testing. In addition, you may be eligible to receive free at-home tests!

We are OPEN for in-person or over the phone visits. Please call our office to schedule an appointment, or for more information on how we can assist you. We do request that you wear a mask when visiting our offices for your safety and the safety of our employees.

Our office hours are 8:30 a.m. – 5 p.m. Monday through Friday. If you have questions or need assistance please give us a call or visit our Contact Us page to send us an e-mail. If you call after hours or we miss your call please leave us a voicemail and we will get back to you as soon as possible.
