CHI is a leader and innovator in addressing barriers to healthcare access, especially with vulnerable populations. CHI is an active participant in the statewide association of similar organizations to advocate for expanded healthcare coverage and to develop best practices.

At CHI we can assist with the application process for health insurance programs and ensure successful enrollment as we follow up with insurance providers to be sure the application is processed successfully. We also ensure all the necessary documentation is submitted along with the application.

We work in collaboration with partner agencies, health care centers, school districts, community colleges, adult education, our workforce, health and human services and businesses to bridge health insurance gaps in our communities. We have skilled and licensed bilingual and bicultural staff who mirror the community’s needs and assist in accessing the services.

Health Insurance Services

Community Health Initiative offers health insurance enrollment and re-enrollment assistance in Covered California, Private Individual and Family Plans, Medi-Cal, Community Health Care Program Northern California-Kaiser Permanente, Medicare, dental plans and vision plans! They also offer outreach and utilization to health insurance programs.

Case Management Services

Once our Health Access Specialists/Assistants have assisted you in enrolling in the appropriate plan, they will follow up with you to make sure your application is approved, that you receive your insurance plan cards, and that you are able to access services. We will contact you twice a year to survey your satisfaction and usage of the insurance plans. When the time comes for you to renew your eligibility, they will assist you in completing the appropriate forms and submitting all required documentation.
